There are 3 steps to the complete the Program Sign-up:

Step 1:  Complete the online form with the basic information below.  Be sure to click on the 'submit' button when complete.  This adds your name to our mailing list and informs the registrar that the printed forms are coming.

Step 2:  Download Registration Form (click here) and the Medical Release / Indemnification Form (click here) .  Be sure to complete and sign both forms.

Step 3:   Please mail the completed forms and appropriate fee to:

Fall 2010 DUES-Remember that the week of summer camp is included in the fall dues.  Club Camp for EDP players and upper club players for this coming fall season will be held 8/9/10-8/13/10 from 9:00-1:00. Players may arrive anytime after 8:30 am.

Your dues help to cover the team’s expenses such as professional and licensed coaches, state registration, league registration, tournament fees, referee fees, camp, insurance, field rental (as of this year we have costs associated with field use).  To help keep the fees reasonable, we use some of the proceeds from our Memorial Day Tournament to offset the dues as well as indoor training costs. Parents are asked to work at the Memorial tournament or find someone to cover their shift. No refunds will be given.
