Welcome to the Program Sign-Up Page

There are 3 steps to the complete the Program Sign-up:

Step 1:  Complete the online form with the basic information below.  Be sure to click on the 'submit' button when complete.  This adds your name to our mailing list and informs the registrar that the printed forms are coming.

Step 2:  Download Registration Form (click here) and the Medical Release / Indemnification Form (click here) .  Be sure to complete and sign both forms.

Step 3:  Tots may bring the competed forms and appropriate fee to the first training session.  All others, please mail the completed forms and appropriate fee to:

Your dues help to cover the team’s expenses such as state registration, league registration, coaching, tournament fees, referee fees, camp, insurance, field rental (as of this year we have costs associated with field use).  To help keep the fees reasonable, we use some of the proceeds from our Memorial Day Tournament to offset the dues as well as indoor training costs.
